The Care Coordinator role has many functions, each of which requires interpersonal commitment and warmth. for What are you going to do? for

• Understand and execute the activities required of the populationsof the community, which includes establishing and maintaining contact and user relationships of the platform through person, telephone and/or email.
• Complete required follow-up calls with contacts as needed for specific populations by performing necessary activities as appropriate.
• Make calls and navigate to the appropriate referral for ongoing care.
• Ensure successful use of motivational interviewing at all times.
• Complete coordination of social care as needed via person, phone, and/or email.
• Accurately document all contact activity using the platform’s contact management system.
• Make sure workflow activities are completed and documented in a timely manner.
• Ensure that the performance of the populations of the community system is seen as professional, safe, competent, responsive and strategically vital by all its clients, both internal and external.
• Attend the ongoing training required by the Signify Community, integrate new skills, and modify client/patient interactions as needed.
• Office at the client’s site (s) and/or at home
• Observe and make recommendations to improve population workflows.
• Continually find local CRG resources added to Signify’s Community Resource Guide. for

We are looking for someone with

• High school diploma or equivalent
• 2 years of experience working directly withend users or patients.
• Proficient computer skills, including web-based browsing, email, and MS Office
• Great ability to navigate and connect customers with community resources.
• Relevance in the payer, the health system and/or non-profit organizations is desired.
• Writing skills: 40 words per minute. for
